-Your account ID (if applicable)
-Your full name (if applicable)
-Your registered birthday as it appears on your account
-Answer to Security Question 1 (if applicable)
-Answer to Security Question 2 (if applicable)
-List at least one game and character name on this account
A picture of your identification. Acceptable forms of identification include:
-Government/State issued photo ID, such as:
-Government Employee ID
-Certificate of Birth/Adoption (only if under age 18).
